medication list for doctor

Anyone know how to put together a medication list?

The doctor said I should start keeping a list of all my medications in my wallet, with dosages and frequency listed.

I take 9 scripts, several are once a day,
twice a day,
3 times a day,
and there's one I take every 2 weeks

Anyway having trouble finding a template that will work.
I've seen 'em in word documents that take up an entire page, but thats too big to fit in my wallet.

so tried printing out a list real tiny at like 3 pt font, which did work but my wife thinks thats too small, so I don't know. I just thought it could be like credit card size laminated I can put in my wallet in case I ever become uncontious or something. My wife thinks thats a good idea, but we're having trouble figuring out how to do that.

Thought maybe an iphone app at first but couldn't find anything remotely close to that so the credit card size card idea was next.
